Click Here for Partnered Agency Application PARENTAL CONSENT FORM If your child is 17 or younger and would like to be a registered volunteer with the Travis AFB volunteer program, please click the Parental Consent Form below, download, complete the form and return to the Airman & Family Readiness Center. They may then register online using the "Youth Registration Form" found under the Volunteer Sign-up/Log-in tab. Just click the "Volunteering at Travis" link to the left of this screen. *Application will be approved once documention has been received and verified. |
Click Here for PARENTAL CONSENT FORM CHILD CARE FOR VOLUNTEERS PROGRAM The Air Force Aid Society (AFAS) recognizes the contributions volunteers make to the Air Force community, thereby offering child care in certain instances. The program will provide, registered volunteers a maximum of 8 hours per week, per child capped at $1,000 per month. AFAS DOES NOT provide this service for anyone volunteering hours for internships or school/work-related training. To verify eligiblity or for more information, contact the Airman & Family Readines Center at 707-424-2486. |
